Student Athletes and Families,
Please review the following documents. All families must fill out the Parent/Guardian Permission Form. In addition, all student-athletes must have had a physical in the last two years of any date in order to participate in athletics. When student-athletes get a physical, they should fill out the physical form with their provider. The Mineral Point High School Athletic Department recommends that students complete their physicals at the end of June or in July. This must be completed every two years. In years when students-athletes do not have a physical, they must fill out an Alternate Year Form. All documents are linked below.

The required participation fee for all sports is $25/sport. This must be paid before student-athletes can compete. For all co-op sports (Cross Country, Soccer, Gymnastics, and Track & Field), student-athletes and their families must pay the participation fee to Dodgeville High School.
